ALUMNI HALL-OF-FAME SELECTION CRITERIA
Identifying the most successful Alumni Members

HOSA Advisors are invited to nominate former HOSA members to the HOSA Alumni Hall-of-Fame. Do you know an outstanding former HOSA member who is now a practicing health professional? If so, please help us share his/her story with today’s HOSA members. In particular, we are looking for careers that are unique since the goal is to highlight multiple careers.


Purpose - The HOSA Alumni Hall-of-Fame is designed to honor outstanding Alumni HOSA members and share health career information with current HOSA members.

SELECTION CRITERIA

Those eligible for inclusion in the HOSA Alumni Hall-of-Fame must satisfy the basic criteria:

• The candidate was an affiliated member of HOSA-National, State and Local. Please indicate the year(s) of membership, the name of the chapter, and the name of the chapter advisor for verification purposes.
• Has been a practicing health professional for three (3) or more years.
• The career represented must not be an exact duplicate of a career already represented in the HOSA Alumni Hall-of-Fame.
• If the alumni member is a doctor or nurse, he/she should write about his/her specialty.
• The candidate must currently be an alumni member of HOSA. Alumni dues can be paid anytime before or during the application process.
• The following information in the template must be supplied to HOSA:
  • Statement answering the question “How did HOSA contribute to your successful health career?” (125 words or less)
  • Description of the career by the professional-not something found in books, but rather, a personal description of what he/she really does. (500 words or less) Note: This information is essential for helping current HOSA members understand what this professional really does on the job and as part of his/her career.
  • List of HOSA Involvement and Activities-including current involvement
  • Education
  • (Optional) More Information-HOSA will do the research necessary to complete this section if not provided.
• A current digital “head shot” photo is submitted in a .jpeg format via e-mail to ksmith@hosa.org, or a picture can be mailed to National HOSA, 6021 Morriss Rd, Suite 111, Flower Mound, TX 75028.
• It is recommended that a second photo of the candidate be provided that shows him or her in an “action shot” in the work environment.

PROCESS

• The candidate must be nominated by a local or state HOSA advisor.
• All additions to the HOSA Alumni Hall-of-Fame must be approved by the respective state advisor.
• Nominations should be submitted via e-mail to ksmith@hosa.org. All information noted under “Selection Criteria” must be included with the application. The nomination may be submitted either by the HOSA advisor or nominee.
• National HOSA reserves the right to edit and reject submissions in an effort to preserve the integrity, appearance, purpose or spirit of the site.
• Alumni members selected for the Hall-of-Fame will be notified and posted on the HOSA web site within 60 days of submission of the application and candidate information.
